Would say she is curious, because she does not understand, what the Doc does to her brother. She probably also did not receive the vaccine so far, otherwise her face expression would be more understanding than curious.
the child behind is showing fear but more importantly is twisting open her own lips to see how it feels, trying to comprehend she is willing to probably pinch hard and pull her own mouth, interesting picture
It’s more concern and worry. There isn’t fear, per say.
